Primary Care
Health Maintenance Organization (HMO): A type of health insurance plan that usually limits coverage to care from doctors who work for or contract with the HMO. It generally does not cover out-of-network care except in an emergency. Irritable Bowel Syndrome
Irritable bowel syndrome (IBS) is a common disorder of the GI tract. It affects about 15% of the US population! Generally, it is characterized by abdominal pain and irregular bowel habits, but exact symptoms will vary from person to person. Some patients will have a lot of diarrhea, others will tend towards constipation, and still others will alternate between the two.

Dr. Kimbrough, a native son of Columbia, obtained his undergraduate degree from Amherst College in Amherst, Mass. Dr. Kimbrough returned to Columbia to obtain his medical degree from USC School of Medicine. He completed his Residency in Internal Medicine and Fellowship in Gastroenterology at Palmetto Richland Memorial Hospital. Dr. Kimbrough is Board Certified in Internal Medicine as well as Gastroenterology.

Colon Cancer
Diverticulosis does not tend to run in families and is not associated with colon cancer. Symptoms from diverticulosis are usually managed conservatively with fiber and other dietary adjustments, sometimes medication such as antispasmodics are useful. There are no known preventative measures for diverticulitis and the consumption of foods with small seeds, peanuts and popcorn does not seem to be a factor, so restriction of these foodstuffs is not usually required.

Most of our procedures are performed at the Columbia Gastrointestinal Endoscopy Center, located at 2739 Laurel Street, Suite 1B; across from Providence Hospital in downtown Columbia.

Stress Management
Figuring out how to best manage IBS can sometimes be a frustrating process. Unfortunately, there isn’t one thing that works for everybody. Instead, finding what works often involves a lot of trial and error. One of our doctors will work closely with you and develop an individualized treatment plan that might include dietary modifications, medications, or even recommendations for stress management.

Dr. Lowman graduated with honors from the University of South Carolina in 1977 with a Bachelor of Science in Psychology. He received his medical degree in 1981 from the Medical University of South Carolina. Following Internships in Psychiatry and Internal Medicine, Dr. Lowman completed a Residency in Internal Medicine at the Medical University of South Carolina. Thereafter, Dr Lowman completed a Fellowship in Gastroenterology at Wilford Hall USAF Medical Center/University of Texas, San Antonio. Dr. Lowman is Board Certified in both Internal Medicine and Gastroenterology.

In order to diagnose fatty liver or “NAFLD (nonalcoholic fatty liver disease)”, other potential problems need to be ruled out. Approaching a patient with an abnormal liver function test usually includes more blood test to rule out things such as celiac disease, iron overload, viral hepatitis, thyroid disease, bile duct disease or autoimmune disease. Sometimes the gastroenterologist must be somewhat of a detective in getting a history of any medications or exposures that may have caused abnormal liver function tests. Proper radiologic imaging is also essential to sealing the diagnosis. At times, a liver biopsy is done to evaluate the cause of abnormal liver function tests as well as to assess the degree of scarring that may have already happened.

The cause of this condition is unknown though it is very much more common in the western world and is thought to perhaps be on the basis of the western diet. Diverticulosis is quite common-more common than not after the age of 50 in the United States. Most people with this have few or no symptoms though cramping and bowel irregularity can be seen. If one of the pouches or pockets becomes infected, this is called diverticulitis. This is rare in the population of individuals with diverticulosis but can be very serious and can require antibiotics and sometimes even surgery. Diverticulitis can be difficult to diagnose, often laboratories and CT scans are used. It should be noted that if diverticulitis is thought present, colonoscopy is best avoided until it resolves on treatment.

Lesions
GERD occurs when the contents of the stomach, which are normally highly acidic, enter the esophagus. Under normal circumstances the stomach lining is protected from the acid that is generated there. However, the esophagus is not equipped to handle that degree of acidity and every time the stomach contents enter the esophagus they can produce symptoms that include heartburn and chest pain. The presence of acid in the esophagus can also lead to injury to the esophageal lining, resulting in lesions like erosions, ulcers and even bleeding. There are also less common or “atypical” symptoms ranging from cough, hoarseness, asthma, sore throat and even accelerated decay of the teeth.
